This link is for brokers requesting appointment with Anthem Insurance Company, Inc. (AICI) to sell Medicare Supplement products in the State of Ohio. AICI requires all advisors to be contracted prior to submitting their first piece of business. AICI has three different links depending on the structure of your agency, and how you want to get paid. Please read the descriptions carefully, and choose the most appropriate contract for your situation.

Broker/ Producer who doesn't work under an agency and/or commissions are paid directly to the Broker/ Producer.

Principal/ Authorized Officer for Agency when request is to set up a new agency. Log in using the agency principal SSN and name. Then indicate the agency name and Tax ID in Section 3 of the Producer Appointment Data Sheet
