Small Group Quoting — May 1 Effective Date Guidelines

Medical Mutual has issued updated submission requirements for agents quoting small groups with a May 1, 2026 effective date. Incomplete submissions will be returned, causing delays — get these details right upfront.

Groups with 1–6 Enrolling
  • FormFire submissions are required
  • Applications are valid for 120 days from signature to enrollment date
Groups with 7–50 Enrolling
  • Renewal increase ≤ 20.9% → Eligible for Simplified Underwriting; submit renewal copy at prescreen
  • Renewal increase ≥ 21% → FormFire required; submit renewal copy via FormFire
  • Submit via email: SimplifiedUnderwriting@Medmutual.com (FormFire also accepted)
  • 7+ prescreens are valid for 90 days from quote generation to enrollment
Required Submission Information

Group Name, Address, Tax ID, SIC Code, Writing Agent/Agency, Current Carrier (virgin groups are ineligible for Simplified UW), total employees (F/T + P/T), enrolling count, membership census, and copy of current renewal.

Keep your census accurate — any changes at enrollment trigger re-underwriting. Do not prescreen more than 90 days ahead of the effective date.
