Anthem SOCA Benefit Plan 2020 1094-B and 1095-C Form Outline

Every employer sponsoring a self-funded plan is responsible for filing forms 1094 (coversheet) and 1095 (individual return) with the Internal Revenue Service (IRS). Employers must complete the 1094-B cover sheet and then file it with the IRS along with the 1095-B form by February 28 if by mail or March 31 if electronically. For the 2020 filing year, Anthem and the SOCA Benefit Plan will be preparing the 1095-B forms for covered groups and providing a 1094-B form for completion. The attached flyer contains additional details on these forms, and how to locate on Anthem’s EmployerAccess Portal. For terminated groups and groups without access to the pre-migration system, please see below.
- If a group terminated at any time in 2020, Anthem is mailing forms to the address and contact last on file. These mailings will begin the week of February 1
- Termed groups who migrated systems in 2020 (i.e. were active on two systems) will be provided with 2 sets of forms (Jan 1-April 30 and May 1-Dec 31).
- Termed groups covered on one system only (no migration) in 2020 will receive one set of forms.
- Service Account Representatives will email any pre-migration forms to the current broker of record for active groups that did not register the pre-migration group number on EmployerAccess.
