UnitedHealthOne Dental insurance offers your clients both substantial discounts from UnitedHealthcare Dental network providers and dental insurance benefits to help pay for preventive, basic, and major dental services. Optional vision insurance is available as a rider to a Stand-alone dental plan in most areas.
Contest Rules:
Contest period is from June 14, 2010, through September 15, 2010. To qualify, a minimum of one application must be issued during the contest period. Plans must be issued by September 30, 2010. Bonus is paid in accordance with the above table. Assignment of prize form is available upon request to transfer cash prize from broker of record to agency. One cash prize per broker, based on his or her personal production. Eligible products include Golden Rule's stand-alone Dental Premier and Dental Value plans only. Availability varies by state. All other health plans for individuals and families do not qualify for the bonus. Qualification is based on production as writing broker. Applications must be new dental clients only; rewrite of existing Golden Rule, American Medical Security, or PacifiCare dental clients do not count toward contest. Applications cannot be split between brokers. Agency business cannot be combined. Any separate applications of family members count as only one application for this contest. All plans must be in force for a minimum of six consecutive months after the effective date in order to count as "issued". If policies are terminated for any reason, the broker will be charged back for any difference in bonus amount paid. All brokers are eligible except for agency codes F1881, IC001 and Sponsored agreement brokers except for Mass Mutual, MetLife, Modern Woodmen of America, South Carolina Farm Bureau, Western-Southern Life Insurance, and Woodmen of the World. 1099s apply. Golden Rule Insurance Company reserves the right to make final judgment on contest qualifiers. Any required disclosures to brokers' clients are brokers' sole responsibility.
